How Managing ADHD with Therapy, Accommodations, and Medication Affects Brain Development

While discussing treatment options with the family of a child newly diagnosed with ADHD, the mother mentioned she'd been reading about the long-term effects of medication on the brain. I braced myself, expecting the usual concerns fueled by misinformation. But instead, she surprised me and said she was fascinated to learn that medication might actually support more typical brain development.
As a parent of a child with ADHD, you may wonder how different treatment approaches affect your child’s development — especially their brain. One common concern is whether using medication changes the brain in harmful or helpful ways. The good news: when ADHD is treated effectively — with a combination of behavioral therapy, school accommodations, and medication when appropriate — it can positively support your child’s development and long-term outcomes.
I want to share what we know about how ADHD impacts the brain, how treatment (especially medication) influences brain development, and what can happen if ADHD goes untreated so you can feel confident in the treatment choices for your child.
Understanding the ADHD Brain
Research shows that ADHD is associated with differences in brain structure and function, especially in areas related to attention, executive function, motivation, and emotional regulation. These include the prefrontal cortex, basal ganglia, and cerebellum. Children with ADHD often have delayed brain maturation—on average, the development of certain brain regions occurs a few years later compared to peers without ADHD, but in the same progression as neurotypical brains.
This delay doesn’t mean their brains are broken—it just means they may take longer to develop certain skills like impulse control, focus, and planning. Support and scaffolding during this time are critical.
What Happens Without Treatment?
When ADHD is not managed, children may experience:
Chronic stress and low self-esteem from repeated failures, negative feedback, and social difficulties.
Poor academic outcomes due to lack of support for attention and executive function.
Increased risk of anxiety, depression, and behavioral issues as frustration and emotional dysregulation go unchecked.

Stimulant medications (like methylphenidate and amphetamine salts) and non-stimulants (like guanfacine or atomoxetine) are often part of ADHD treatment. Contrary to fears that medication might "dull" or damage the brain, studies show that:
Medication normalizes brain activity in key regions responsible for attention and impulse control.
Stimulants improve dopamine and norepinephrine function, which helps with focus, motivation, and executive function.
Long-term use of medication is not associated with negative effects on brain growth. Medication may help brains develop in a more typical pattern.
Brain imaging studies suggest that children with ADHD who take medication over time show brain development patterns more similar to children without ADHD. This doesn’t mean medication "fixes" ADHD, but it can support more effective functioning during critical periods of brain growth and learning.
The Power of Therapy and Accommodations
Medication is not a magic fix. The most effective treatment plans combine medication with:
Behavioral therapy, which helps kids learn coping strategies, organizational skills, and how to manage emotions.
Parent training, which empowers families to support positive behaviors and reduce conflict.
School accommodations, like extended test time or movement breaks, which level the playing field and reduce unnecessary frustration.
These strategies reduce stress, promote confidence, and give kids the tools they need to succeed — socially, emotionally, and academically. They also support the development of executive function skills, which are essential for independence.

The goal of treatment is not just symptom management — it’s helping your child grow into their best self. Untreated ADHD can interfere with learning, relationships, and identity development.
Well-managed ADHD allows children to:
